Louisiana becomes 11th state to benefit from unique model of care

Residents of long term care facilities owned and operated by CommCare Corporation and Nexion Health will now benefit from access to an enhanced level of proactive, coordinated healthcare through a new partnership announced today between American Health Plans, CommCare Corporation, and Nexion Health. In addition to this partnership, American Health Plans* has acquired Louisiana-based Dignity Health Plan, a Medicare Advantage plan serving Louisiana.
The partnership, which will be implemented in 29 homes across the state of Louisiana, will give residents of participating CommCare and Nexion facilities the opportunity to join American Health Plans’ provider-owned Institutional Special Needs Plan (ISNP). Jason Haney, senior vice president of operations for American Health Plans, commented on the partnership saying “CommCare’s rich history of quality care makes this an ideal partnership. They committed to providing residents with best-in-class care by launching an ISNP and we are excited to help them expand with new products and services.” These Medicare Advantage special needs plans specifically cater to Medicare recipients who live in skilled nursing facilities. The plans deliver a coordinated care model that addresses all needed medical treatment and preventive services supported by TruHealth, an integrated care team that is led by a nurse practitioner or physician assistant to provide individualized health care services.

American Health Plans, the parent company of American Health Advantage of Louisiana, is now operating in 11 states. The Franklin, Tennessee-based health care company operates the largest provider-owned ISNP platform in the nation and has plans to expand to Indiana in 2024. The unique ISNP model of care improves health outcomes by bolstering medical services at the point of care while also reducing inefficient health care spending. “Partnering with American Health Plans can allow independent ISNPs to leverage our national platform,” Haney said. “In Louisiana we have already seen the benefits of improved service delivery, lower costs and the ability to grow membership with new partners.
