American Health Plans Announces Expansion into Louisiana with CommCare and Nexion Health Partnership

Residents of long term care facilities owned and operated by CommCare Corporation and Nexion Health will now benefit from access to an enhanced level of proactive, coordinated healthcare through a new partnership announced today between American Health Plans, CommCare Corporation, and Nexion Health. In addition to this partnership, American Health Plans* has acquired Louisiana-based Dignity Health Plan, a Medicare Advantage plan serving Louisiana.

The partnership, which will be implemented in 29 homes across the state of Louisiana, will give residents of participating CommCare and Nexion facilities the opportunity to join American Health Plans’ provider-owned Institutional Special Needs Plan (ISNP). Jason Haney, senior vice president of operations for American Health Plans, commented on the partnership saying “CommCare’s rich history of quality care makes this an ideal partnership. They committed to providing residents with best-in-class care by launching an ISNP and we are excited to help them expand with new products and services.” These Medicare Advantage special needs plans specifically cater to Medicare recipients who live in skilled nursing facilities. The plans deliver a coordinated care model that addresses all needed medical treatment and preventive services supported by TruHealth, an integrated care team that is led by a nurse practitioner or physician assistant to provide individualized health care services.
