Residents of participating Cascadia Healthcare facilities in Idaho and Utah will now benefit from accessing an enhanced level of proactive, coordinated healthcare through a new partnership announced today between American Health Plans and Cascadia Healthcare. Cascadia Healthcare joins Cascades, Mission and Sunshine Terrace in offering a specialized Medicare Advantage plan to qualified long-term care residents across Idaho and Utah.
The partnership will give Cascadia residents the opportunity to join the American Health Plans provider-owned Institutional Special Needs Plan (ISNP). These Medicare Advantage special needs plans specifically cater to Medicare recipients who live in skilled nursing facilities and qualify for certain levels of health care services. These plans deliver a coordinated care model that addresses all needed medical treatment and preventive services.
